How to register

 

We are using a new online 'Register with a GP Surgery' service that makes it easy to register with the surgery.

Just fill in this quick online form to start the process.

You don't need proof of address or immigration status, or an NHS number.

The service is designed and run by the NHS, so your personal information is safe.

It reduces our administrative workload and also reduces errors, making it easier for everyone.

Anyone in England can register with a GP surgery and it's free to register. We are a motivated, caring team who endeavour to provide you with excellent patient-centred service under our care. Please explore our website for information on appointments, the services we provide, our staff, health advice and links to online health resources.

If you would prefer to visit the surgery in order to register with us, we would be grateful if you would bring proof of ID and address  - it will help if you have one or more of the following:

  • passport
  • driving license 
  • birth certificate
  • a utility bill (within last 3 months)
  • a bank statement (within last 3 months)

If you are unsure if you live within our area ask at reception or use the postcode boundary checker. If you decide to register with us, you will be asked if you are within our catchment area, as we are currently not taking out of area registrations. Important - We require your previous medical records before prescribing regular medication. Therefore please ensure that you have a months supply of regular medication from your current surgery before registering.

Please note that if you move out of our catchment area you will be required to register at a surgery nearer your new address.

Frequently Asked Questions

  • What happens after submitting my registration? Once you have completed your registration using the Register with Dean Cross Surgery function, this will electronically be sent to the surgery, and we will register you with the practice within 7 working days of receiving the request. If there are any issues with the registration, one of the team will be in contact with you within that same timescale. If you register with us in person, we also aim to register you with the practice within 7 working days.
  • Once I am registered at the surgery, how do I make an appointment? Since October 2023 we have started to use a telephone triage system called Anima, which patients can access on their mobile phones, PC or tablet - this is known as an online consultation and many of the things that you would have previously had to contact the practice for by phone or in person, can be done by Anima. The benefits include easier access - you are able to submit a request from your location without having to wait in a telephone queue, or visit the practice. It should only take a couple of minutes to fill out your information and your request will be seen by the right person in the practice and responded to by the right person in the practice. Sending your request via Anima allows the telephone lines to be freed up for the most poorly patients or patients who are not able to access online services.
  • What if I don't have access to the internet or struggle to complete the Anima form? Please be reassured that you can still telephone us or come in to the practice and our team will fill an Anima request in on your behalf.
  • Does this mean that I won't see a GP anymore? Not at all - completing the form online is just a preliminary way of collecting information to ensure that you are seen by the right person, at the right time. You may still be contacted by a GP or it could be another person from the practice who can better meet your needs. 
  • Can reception make me an appointment instead? Even if you visit the surgery in person or contact us by phone the Reception Team will not be able to book a routine GP appointment. They will assist you to complete the online Anima form if you need help.
  • How do you get an Anima Account? We suggest that once you receive notification from us that you are registered you set up an Anima account. To set up an account, please click here.
