If you live within our practice area you are welcome complete a new patient registration form and bring to reception where our staff will be happy to guide you through the procedure. Eligibility can be quickly confirmed from your address. We may ask for proof of identity by way of a recent utility bill and one form of photo ID.
To be eligible to register at the practice, you must live within the practice catchment area.
We must now confirm the identity of patients that register with a form of photo identification.
Practices are required under the GMS contract, to allocate a named, accountable GP to all patients, including children. All new patients registering with the practice will be informed about their named GP at registration. Please ask your doctor, or the reception staff who your named, accountable GP is. Should you express a wish for this to be changed to another GP we will do our best to accommodate your wishes.
As it often takes some time for records to be forwarded from your former practice all newly registered patients will be asked to complete a health questionnaire and are offered a consultation with the nurse.
